General
We accept Aetna, Anthem, Medical Mutual, and SummaCare.
Every insurance company has a contracted rate. Once insurance is verified by our billing department, you will be notified of your out of pocket responsibility.
Payment may be made with Visa, MasterCard, HSA, FSA, and cash.
We request that all appointments are cancelled or rescheduled at least 24-hours in advance. Otherwise, a fee of $100 will be charged to the card on file unless prior arrangements have been made.
